I have been honored to have been entrusted with providing care to tens of thousands of people since September of 1983 as well as teaching and mentoring podiatric medical students in state-of-the-art foot surgery. I have had the privilege of knowing and becoming good friends with my patients and office and hospital staff who trusted in me. I thank all of you for touching my life.

If you need a copy of your office notes created by your previous visits with me, you will need to send the medical records' request form for Dr. Leavitt, attached to this site, along with a check for $15.00 to Cindy Berube at 19 Timber Lane, Methuen, MA 01844.
If you need a copy of your x-rays, please send the attached form along with the request fee to New England Baptist Hospital, X-Ray Department, 125 Parker Hill Avenue, Boston, MA 02120.
If you need a copy of records generated at the Hospital such as operative notes, anesthesia records, etc., please see the attached form that you will complete and send to the medical records department at the Hospital, Medical Records, New England Baptist Hospital, 125 Parker Hill Avenue, Boston, MA 02120.

Dr. Leavitt specializes in surgical correction of foot problems. He places a high priority on learning and using the most up-to-date procedures to benefit patients. Dr. Leavitt is a Diplomate, American Board of Foot and Ankle Surgery, certified in Foot Surgery, and has been in practice treating disorders of the foot, ankle joint and lower leg since 1983.
